I am a fully qualified Occupational Therapist. I am a member of The Royal College of Occupational Therapists (RCOT)) and registered with the Health and Care Professionals Council ( HCPC).
Data collected is held on the grounds of being for legitimate business interests or to fulfil a contractual obligation.
How and what information is collected about you
We may collect the following information from you via email, telephone call, post, questionnaires, forms, or in-person:
· Name
· Date of Birth
· Contact details (phone, email, address)
· Parent/Guardian names and contact details (phone, email, address) for minors
· Personal medical and lifestyle information, including reports and letters from other healthcare professionals
· Education setting details
What we do with the information we gather and what it is used for
· Professional record keeping of Occupational Therapy services provided to clients.
· Provision of Occupational Therapy services.
· Sharing your information with relevant parties when necessary, with your consent, such as other Health and Education professionals.
· Your data will not be transferred to a country outside the European Union.
Following completion of your child's Occupational Therapy service, we retain your personal data for the period defined by our professional association RCOT and registrant body, Health and Care Professions Council. This enables us to process any complaint you may make. In this case the legal basis of our holding your personal data is for contract administration.
I act as a Data Controller of your information and undertake to protect your personal and sensitive data in a manner that is consistent with the requirements of the Data Protection Act/General Data Protection Regulation (GDPR). I will take reasonable measures to ensure the secure storage of your data.
We ensure the information we hold is kept in secure locations, restrict access to information to authorised personnel only, protect personal and confidential information held on equipment such as password protected laptops. Any written information recorded during observation are non identifiable and are disposed of once the information has been transferred onto a digital file.
YOUR RIGHTS
Data protection legislation gives you, the parent/carer, various rights. The most important of these are as follows: – You have the right to a copy of information we hold about your child – You have the right to ask for your record to be amended if you believe that it is wrong.
